RETURN FORMS TO UVM EXTENSION
Forms may be mailed to or dropped off at the UVM Extension office at 271 North Main Street, Suite 110, Rutland, VT 05701. There is also a black drop box to the right of the office entrance that forms may be dropped in. Please make checks payable to: Rutland County 4-H Foundation. All forms and signatures are required to process this entry.
